STUDENT ENROLLMENT
When enrolling a new or transfer student, follow the steps below to ensure a smooth transition. (A checklist is provided for your convenience.) Once you have turned in all of the required paperwork, allow 5-10 days for the enrollment to be processed.
To enroll in school, a new student must be:
A resident of Northeast Bradford School District (proof of residency is required)
5 years old on or before Sept. 1 of the intended school year (for kindergarten)
Immunized to meet Pennsylvania’s requirements
ENROLLMENT PROCESS
Step 1: Download and print the applicable forms. Or you may pick up the enrollment package from the district office.

Step 2: Ensure your child has the state-mandated immunizations and physical/dental exams.
IMMUNIZATIONS
The state requires students attending public school receive immunizations to prevent the spread of communicable disease.
For students in ALL grades:
4 doses of tetanus, diphtheria, and acellular pertussis (1 dose on or after the 4th birthday)
4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)
2 doses of measles, mumps, rubella (German measles)
3 doses of hepatitis B
2 doses of varicella (chickenpox) vaccine or evidence of immunity
For students entering grade 7:
1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
1 dose of meningococcal conjugate vaccine (MCV)
For students entering grade 12:
1 dose of meningococcal conjugate vaccine (MCV)
MEDICAL / DENTAL EXAMS
Students entering kindergarten, grades 6 and 11 require a physical exam. Students entering kindergarten, grades 3 and 7 require a dental exam. The required forms to take to the doctor and dentist are available below.

If the student is transferring from another school, the NEB school nurse will screen the transfer documents to ensure all requirements have been met.
Step 3: Gather the required documents to accompany the enrollment forms: birth record, immunization record, and proof of residency.
Original or certified copy of birth certificate, or original baptismal record.
If you do not have an original birth record, you may request a certified copy from the Vital Records Office of the state where your child was born.

Immunization records
Bring a copy of your child’s most recent immunization record. (See immunization requirements above.)
Proof of residency
The state requires parents to provide the district with proof of residency. This can be any document that shows your name and street address to indicate you live in our district. Examples are driver’s license, vehicle registration, lease, current tax bill, current utility bill, or current credit card statement.
If you don’t have proof of residency in your name (i.e., you’re living in someone else’s home), you may provide a notarized affidavit or letter of residency, signed by the landlord or legal owner of the property.

Step 4: Please call 570-744-2521 extension 2265 or email vperez@nebpanthers.com to set up an appointment. Make sure to bring the completed forms and required documents on your appointment day. The enrollment process takes 5-10 business days to complete once all forms and documents have been received.
ENROLLMENT IN ONLINE ACADEMY
To enroll in Panther Academy, students must first withdraw from their current school. If attending one of the Northeast Bradford schools, download and complete the Panther Academy Withdrawal-Enrollment Form. A parent is required to deliver the withdrawal form to the school. Once a student is withdrawn, follow the new student enrollment process.
To withdraw from Panther Academy and enroll in a Northeast Bradford school, complete the Panther Academy Withdrawal-Enrollment Form.
